Trail Winds Hospice is currently hiring Full-Time RN Case Managers and On-Call RN’s.

We are looking for high-energy, compassionate, detail-oriented RN’s to be Hospice Case Managers for our patients. Our Hospice RN Case Managers plan, organize, and direct hospice care utilizing the nursing process of assessment planning, interventions, implementation, and evaluation; and effectively interact with patients, caregivers, families, and other interdisciplinary team members while maintaining standards of professional nursing and clinical competency. Qualifications:

· Associate’s or Bachelor’s degree in nursing.

· Holds a current unencumbered Colorado license as a Registered Nurse.

· Minimum one (1) year experience as an RN Hospice/home health experience preferred but not required.

· Must be computer proficient in typing and various programs, including a background in EMR systems.

· Possess and maintains current CPR certification.

· COVID-19 Vaccination. CMS requires that all patient-facing staff, as well as those that are in contact with patient-facing staff, be fully COVID vaccinated, or have a documented religious or medical exemption. This is required for all healthcare organizations that receive federal funds through Medicare or Medicaid.

· Annual Flu Vaccination

· Current valid driver’s license, auto insurance, and a reliable car.

CNA

Required License & Education

· Active Certified Nursing Assistant License in Colorado

· In good standing with DORA

· 1-2 years CNA experience preferred, hospice preferred

CNA Job Role:

The CNA – Certified Nursing Assistant will provide personal care to patients under the direction of the licensed nurse supervisor.

· Assists patients in their homes and/or facilities.

· Assists patients with personal care including bathing, grooming, hygiene, and placement of adaptive equipment

· Ensures clear communication with RN about patient needs

· Reports all changes in patient condition to supervisor as soon as possible

· Delivers supplies to patients’ homes and facilities regularly

Social Worker

Trail Winds Hospice is currently hiring a Part-Time Social Worker.

 

Responsibilities:

The licensed social worker administers social services to patients requiring intermittent professional services and teaches the patient, family, and other members of the health care team. These services are performed in accordance with the physician’s orders and the established plan of care.

 

Qualifications:

· Must have a Master’s of Social Work

· Minimum one (1) year of social work experience, preferably in hospice.

Hospice LPN

The Licensed Practical Nurse is responsible for providing direct patient care under the supervision of the RN Case Manager.

Responsibilities/duties, Skills & Experience:

· Demonstrates the ability to contribute to the organization’s vision and mission; protects confidentiality

· Maintains appropriate personal and professional boundaries. Interacts effectively as a team member and works in a dependable manner

· Communicates clearly, both verbally and written. Capable of detailed follow-through in a timely, responsible manner

· Demonstrates proficiency with computers, copiers, fax machines, scanners, and other standard office equipment

· Demonstrates intermediate knowledge with Google Docs, Google Drive, email

· Demonstrates basic knowledge of EMR Systems

· Demonstrates excellent customer services skills in person or on the telephone, with our customers and internal departments

· Applies critical thinking and problem-solving appropriate to the role

 

 

Qualifications:

· Active LPN license in CO

· Minimum Experience: 1-2 years hospice experience

· Required License: LPN

 

Requirements:

· TB testing annually

· COVID-19 Vaccine

· Background check

· Read and adhere to all agency policies and procedures including the employee handbook and HIPAA Program
